Your First Visit

Upon entering the office, We have some brief paperwork for you to fill out, or if you'd like to get a jumpstart on that paperwork, you can download it by clicking the button below. Return completed forms via email (jensenchiroclinic@gmail.com) before your visit or print it out and bring it with you. Otherwise, we will have the forms ready for you when you arrive. If you have insurance or Medicare, please bring your insurance card or cards and photo ID for us to copy.

The doctor will take a complete history, give you a physical examination and will recommend treatment options specifically for you and your condition.

X-Ray studies and/or labs will be taken if warranted.

You will receive same-day treatment.

Home instructions will be sent with you if necessary.

Your next appointment will be scheduled if needed.
